tip 2Louisiana birth, marriage and death certificates must bear the signature of the Registrar of Vital Records.
 
tip 2Adoption papers, diplomas and transcripts, powers of attorney, etc. must be notarized by a Registered Notary in the Commonwealth of Kentucky. The signature must be certified by the county clerk in the county where the Notary took the oath of office.
 
tip 2Notarized documents must have complete Jurat/acknowledgment and printed notary's name under the signature.
